This rebuild’s public appointment form collects only logistics: name, email, phone, a preferred window, and an optional note. It does not ask for symptoms, date of birth, medical history, or insurance identifiers. Do not type those into the note field.

Submissions are used to return your call or email about scheduling. If a webhook is configured on the host, the same logistics payload is sent there. We do not load a Facebook pixel, Google Tag Manager, or reCAPTCHA on these pages.

For clinical records, billing, and marketing lists attached to the live Squarespace property, follow the policy on thelegacyclinic.com and contact jennifer@thelegacyclinic.com. This site is a performance rebuild hosted on h.ai infrastructure for preview; it is not a replacement medical-records system.
