Chronic pain in the Bighorn Basin is usually a mix of load, old injuries, sleep debt, and inflammatory noise. The Legacy Clinic does not run a procedure mill and does not advertise miracle weeks. Dr. Thoma’s first job is to decide whether the pain pattern is something this clinic should touch, something that needs imaging or a medical colleague, or something whose main driver is not a joint at all.
The functional exam includes orthopedic and neurologic screens for a reason. Numbness, progressive weakness, unexplained weight loss, night pain that wakes you, or bowel and bladder change are not “request an appointment” material — they are same-day medical material. For the rest, Day 1 builds a baseline and Day 2 explains what we think is driving the pain you can live with long enough to get to Cody.
Pain relief here might mean an adjustment, acupuncture, a nutrition change that lowers inflammatory load, a sleep plan, or a referral. It will not mean a public quiz that spits out a protocol. It will not mean collecting your pain scores in a Squarespace box that sits on a third-party server with a Facebook pixel. This rebuild does not load a pixel. The live Squarespace site does. That is one reason these pages exist.
What “relief” is allowed to mean
A good outcome is function: sleep through a night, lift a grandchild, finish a shift. A bad outcome is a patient who is still here every week without a story for why. The case manager will talk about visit frequency in plain numbers after the report of findings. If we cannot explain the plan in language you can repeat to a spouse in the parking lot, the plan is not done.
Medications you already take stay with the prescriber who wrote them. We do not taper controlled substances from a chiropractic office over email. We will look at how those medications sit next to sleep and gut findings so your other physicians are not working blind — with your records, not with a web form.

Pain, opioids, and the other physicians in your file
If you already have a pain-management physician, we are an adjunct or we are a second opinion, not a stealth taper clinic. Bring the medication list on intake. We will not collect it in a browser. We will not tell you to stop a medication because a blog said so.
Workman’s comp and disability paperwork can consume a one-doctor clinic. If that is your primary need, say it on the phone so we can tell you whether we are the right room. Honesty here saves you a drive from Worland.
Flare plans belong in writing: what you do at 9 p.m. on a Saturday when the clinic is closed. That plan should include red-flag instructions, not a hope that Dr. Thoma will see a portal message in the night. He will not. Hours are hours.
Outcomes we will celebrate: sleeping, lifting, showing up to a shift. Outcomes we will not put on a billboard: “pain-free forever.” Anyone who promises that on a Cody highway sign is selling something this clinic is not.
Pain also lives in the week: a kitchen shift, a freeze-off, a long sit in a pickup, a night that never went deep. The structural exam and the lifestyle questions are supposed to meet on Day 2 so we do not treat a disk and ignore the sleep that keeps recreating it. That meeting is why this is not a five-minute adjustment mill.

- Do you prescribe pain medication?
- No. Dr. Thoma is a chiropractic physician. Medication changes belong with the clinician who prescribes them.
- Can I come only for a single pain visit?
- New patients start with the two-day exam so we are not treating a mystery. Established patients can be scheduled for focused visits after that.
